this is gonna get closed or edited really quick, but what u gotta do is get a hex editor and find :
"00 74 4B E8 CA FD FF FF 85 C0 7D 06 33 C0 50 50 EB 44 F6 05" then change the "7D" to "EB" to make it this :
"00 74 4B E8 CA FD FF FF 85 C0 EB 06 33 C0 50 50 EB 44 F6 05" then it will work A OKAY

Whacking those two 00 bytes from the before and after sequences seems to work better. See how I quoted it.

Never Games that have been released have the media Flag Moved from one location to another so your mod-chip gets confused. If you simply apply the patch that Xeero has detailed above your game will work. Enjoy.

Guys, I've tried this and I've had problems with some 'newer' games sitll not working. Do I need a newer bios or anything to get these puppies going?

I'm kinda stumped :

Should I not be using Xiso to extract the iso dealies?
I heard there was a problem with that somewhere

the patch is sometimes needed in more than one file, look for xbe files and go from there

Or (as recommended) check the ISO for multiple occurrences.
