Display Gamer Picture, Customising the picture on your gamertag Options

I've searched for this on the FAQ, but can't find the definitive answer;

This is what I think I know;
1. Custom gamerpictures are only available if you take a photo with your 360 vision camera
2. They are only displayed to your friends.

My questions are - how come almost every member on this site seems to have a custom picture in their gamer tag, and how do I create/load mine?

'Cos most of those don't look like they were taken with a camera.

Please excuse my ignorance and my apologies if it's been dealt with somewhere else. I tried searching the forums but it didn't seem to work.
