I accidently reformatted my xbox 360 slim 120gb HDD. How do I format it back so my slim can read the hard drive again? I would also like to add my backup gamertag I saved to my desktop before the accident hdd wipe. Where Do I add it?

simple answer is no you cant!!

the hdss has been overwritten and there are no tools to rewrite it on an official hdd

buy a western digital hdd and use hddhackr to make it into an official hdd

then use xplorer360 or horizon to load your backed up info onto it
